About the service

The Type 2 Diabetes Food and Health Service is part of the Nutrition and Dietetic service supporting people across Highland Health and Social Care Partnership who are living with pre-diabetes and type 2 diabetes.

The service helps people explore their relationship with food and health in a supportive and non-judgemental way to make meaningful changes.

We recognise that managing diabetes and wider health outcomes are influenced by life circumstances such as social, emotional and environmental factors, as well as lifestyle.

We offer practical support, education and simple strategies to help you feel more confident in managing your condition and improving your overall health and wellbeing.

Eligibility and access

You may be eligible for this service if:

  • you have been diagnosed with pre-diabetes and type 2 diabetes within the last 6 years

  • and/or your HbA1c is 42 mmol/mol or above

To access the service, please speak to your GP or Practice Nurse who can make a referral on your behalf.

We provide a range of flexible support options to suit your needs:

  • face-to-face group education - in selected locations

  • remote group education - join from the comfort of your own home

  • one-to-one support – if a group setting is not suitable

  • digital appointments - via NHS Near Me

  • telephone consultations

What happens after referral?

You will be invited for a pre‑assessment telephone appointment with a Community Food and Health Practitioner. At this appointment, we will:

  • talk through your medical history

  • explore eating habits and lifestyle

  • discuss your goals and what you would like to get from the service

This helps us understand your needs and make sure the support we offer is right for you.
