Dementia services - Argyll and Bute

The Consultant Psychiatrist is medical practitioner with specialist training and the most senior clinician in the service. They will provide clinical leadership, support and supervision to members of the multidisciplinary team in addition to direct clinical work. They may be required to exercise statutory functions related to relevant mental health and incapacity legislation.

The Clinical Services Manager provides strategic and operational leadership and is responsible for the delivery, development and quality of the Enhanced Community Dementia Service.

The Team Leader provides day-to-day clinical and operational leadership, supporting the multidisciplinary team to deliver high-quality, person-centred care to people living with dementia and their families and carers.

The Mental Health Advanced Nurse Practitioner is an experienced practitioner working autonomously and in conjunction with other health care professionals to assess, diagnose, plan, treat and manage the care of patients with non-complex presentations and needs.

During episodes of care, to be responsible as the case coordinator for the assessment, planning, implementation and care coordination to people affected by dementia. To provide specialist, professional advice and support to other agencies and members of the multidisciplinary team.

The Occupational Therapist provides direct clinical care, which encompasses occupational therapy functional and environmental assessment and intervention of patients within the specialist area of dementia. To offer crisis intervention, low intensity psychological interventions, and evidence based therapeutic interventions.

The Social Worker conducts assessments and review of care, responding to risk and protection issues, development of care packages. Social workers may hold some cases in their own right but will also act as the specialist link with social work colleagues who will be supporting adults within communities across Argyll and Bute.

To assist and support members of the multidisciplinary community dementia service, in collaboration with service users, in the overall provision of best quality care within a recovery framework, whilst under direct and indirect supervision. Clinical supervision to the health care support workers will be provided by the Advanced Nurse Practitioner ANP and Team Lead.

The post-diagnostic support Link Workers provide information, advice, cognitive stimulation and support for people who have been recently diagnosed with dementia, their carers and families, and assists the person and their family to come to terms with their condition.
